12th Grade Immunization Requirements
Hello Parents and/or Guardians!
In addition to school entry immunization requirements, students entering the12th grade in North Carolina are required to show proof they have received:
a second dose of Meningococcal Conjugate ACWY vaccine.
At the beginning of the school year, all 12th grade parents/guardians need to submit an updated immunization record to the school indicating your child has received the required vaccination.
According to North Carolina law (General Statute 130A-155), your child will not be allowed to attend school if proof is not presented within the first 30 days of attendance.
The required vaccine and other recommended vaccines may be obtained at your child’s regular healthcare provider

Walk-In Clinic:
Monday 8 am - 4:30 pm
Tuesday 8 am - 4:30 pm (2nd Tuesday 8 am-6:30 pm)
Wednesday 8 am - 4:30 pm
Thursday 8 am - 4:30 pm
Friday 8 am - 12:30 pm
On very busy days the clinic may reach capacity before posted closing times.
