Parent Permission Slip for Student Dismissal After EOC/CTE Testing

Attention Students, Parents/Guardians,

 

During the week of March 8-12, 2021 students will be on campus in order to complete their End-of-Course (EOC) testing and Career and Technical Education (CTE) Post-Assessments. Buses will transport students to and from school. In the near future, additional information will be sent out regarding their bus transportation information. Students will be permitted to be picked up from the school once their room’s testing session has completed. Parents/Guardians will NOT be permitted to enter the building to check out their student. If you would like to check your student out once testing has completed, please fill out the information below, scan a picture of the form and email to Classie McConago at (classiemcconago@ccs.k12.nc.us) NO LATER than Friday March 5, 2021 at 4:00PM. Please list any adult that has permission to pick up your child the day of testing. Your student WILL NOT be released to anyone NOT listed below. The person picking up your student MUST HAVE photo ID in order for us to release your child. You must have a parent or guardian signature on this page, or your request will not be honored. Your signature on this form releases Douglas Byrd High School and Cumberland County Schools from any liability once released to the individual(s) listed below. When you arrive on campus, turn into the second parking lot (student parking lot) and park along the curb, stay in line. A Douglas Byrd High School employee will come to your passenger window, ask for the student’s name, your name and your ID. Once it has been confirmed with the front office, your child will be sent to the car as long as their testing session has ended. Wednesday March 10, 2021 - 3rd period exams
