REQUIRED DOCUMENTS

Valid government I.D. for the head of household

Birth certificates for all children being registered OR official custody papers if applicant is not the biological parent

Proof of residence - ONE of the following in applicant’s name

- Current lease

- Mortgage

- Notarized letter of residency: (if living with someone)

- Proof of Need - ONE of the following in applicant’s name

- Social security award letter, declaration letter, SNAP or TANF

IF YOU DO NOT RECEIVE THE ABOVE GOVERNMENT ASSISTANCE

-Provide proof of ALL income from August (pay stubs, social security, child support, workers comp, etc.)
